Insurance Reimbursement

NutriVida Nutrition Therapy is a private-pay only practice. NutriVida Nutrition Therapy does not accept insurance and/or manage any insurance issues. It is encouraged you contact your insurance provider and verify if you qualify for any insurance reimbursement.

Helpful questions to ask your insurance provider:

• Does my insurance plan cover any of the following: outpatient nutrition counseling, medical nutrition therapy, preventative nutrition counseling, dietary counseling and monitoring?

• Is there a deductible I must meet first? Is there a maximum amount I must pay out-of-pocket first?

• Is a referral required?

• Are there any specific diagnosis code restriction or requirements?

• How many sessions (or units) am I eligible for?
